Sermorelin before and after is better understood as a lab-and-measurement timeline than a pair of photos. Small adult studies found that growth-hormone signals could change within two weeks, while body-composition measures either did not change by six weeks or showed one narrow, sex-specific result after 16 weeks. No trial tested facial appearance, wrinkle reduction, or a dramatic physical change. The honest expectation is simple: early changes, if any, are more likely to be measurable in blood than visible in a mirror.

What sermorelin does before anything looks different

Sermorelin is a GHRH analogue, a small copy of the signal that tells the pituitary gland at the base of the brain to release growth hormone. It does not supply growth hormone itself. The response still depends on a pituitary that can answer the signal.

Growth hormone comes in pulses. It also prompts the body to make IGF-1, a steadier blood marker that carries some of growth hormone's downstream signals. A change in either number shows biological activity. It does not show that fat, muscle, sleep, or appearance changed.

The sermorelin before and after timeline in adult studies

The adult research is tiny. The two most useful papers studied 11 men in one case and 19 men and women in the other. They did not test a current compounded formulation against a modern control group.

Around two weeks: In a 1997 study of 19 adults ages 55 to 71, IGF-1 and its main binding protein increased within two weeks. The study used a modified GHRH(1–29) analogue, meaning it was closely related to sermorelin but one of its protein building blocks had been swapped. Those lab changes lasted through week 12 and were moving back toward baseline by week 16.

At six weeks: A second 1997 study followed 11 healthy men ages 64 to 76 who received unmodified GHRH(1–29), the same short fragment sermorelin copies. Overnight growth-hormone release increased, but IGF-1 did not. Neither did weight, waist-to-hip ratio, or fat and lean tissue on a DEXA scan, an X-ray test that estimates body composition. Two of six strength tests and one endurance test improved, but there was no untreated comparison group. The Vittone study in Metabolism therefore shows a hormone response without a matching visible body change.

At 16 weeks: In the 19-person modified-analogue study, body weight stayed unchanged. Skin thickness increased in both sexes, and lean mass—everything in the body except fat and bone—increased in the nine men but not the 10 women. The study found no other body-composition or bone-density change. The Khorram trial in The Journal of Clinical Endocrinology & Metabolism is useful, but it cannot supply a reliable month-by-month forecast for a different formulation or a younger adult.

CJC-1295 with ipamorelin is sometimes considered for the same general growth-hormone pathway. It uses a different combination of signals, so its studies cannot fill the gaps in the sermorelin timeline or make the result more predictable.

What “sermorelin before and after face” can honestly mean

People who search about the face are usually asking about wrinkles, firmness, under-eye changes, or collagen. The adult trial does not answer those questions. Researchers used calipers to measure skin thickness at the hands and forearms. They did not measure the face, collagen production, wrinkles, hydration, or how participants looked.

Thicker skin is a measurement, not a cosmetic verdict. It could be worth studying further, but it is not evidence of facial rejuvenation. Lighting, camera distance, expression, hydration, and other treatments can all change an online photo without showing what sermorelin contributed.

Men and women did not have the same measured result

The modified-analogue trial found an early IGF-1 rise in both sexes. At 16 weeks, however, the lean-mass finding appeared only in the nine men. The 10 women did not show it. A group that small cannot establish that all men respond or that women never do; it only tells us what happened in that trial.

Why online before-and-after photos are weak evidence

A photo rarely controls the things that matter. Training, food intake, sleep, weight-loss medication, hormone treatment, pose, and time can all change between frames. The picture also cannot show whether IGF-1 moved or whether the vial contained what its label claimed.

A more useful timeline starts with the reason a clinician considered treatment and uses the same measure at baseline and follow-up. That could mean an age-adjusted IGF-1 value, the same body-composition method, or a specific functional measure. The goal is not to collect every number. It is to avoid mistaking a feeling or a flattering photograph for a drug effect.

What the regulatory history changes

Geref, a sermorelin acetate product for certain children with growth-hormone deficiency, received U.S. marketing approval in 1997. The agency withdrew its approval effective June 18, 2009, after the manufacturer requested withdrawal. A later Federal Register notice says the products were not withdrawn for reasons of safety or effectiveness.
