What happens when you stop taking semaglutide is usually gradual, not a sudden withdrawal reaction. As the medication clears over roughly five weeks, its effects on appetite, fullness and digestion fade. Hunger may become more noticeable. Weight regain is common, and improvements in measures such as blood pressure and blood lipids may move back toward baseline. That pattern reflects the biology of chronic weight regulation, not a failure of character.
Stopping does not guarantee that every pound returns, and it does not produce the same result for everyone. The clearest evidence comes from randomized withdrawal trials, which report group averages rather than individual forecasts.
What happens when you stop taking semaglutide, week by week
Semaglutide has an elimination half-life of approximately one week, according to a randomized pharmacokinetic study published in Advances in Therapy in 2018 (Ikushima et al.). After one half-life, about half the exposure remains; after roughly five, only a small fraction remains. The detailed arithmetic belongs in our guide to how long semaglutide stays in your system.
During those first several weeks, medication-related appetite control and delayed gastric emptying do not switch off at a single moment. They become less pronounced as exposure falls. A small randomized trial found that semaglutide was associated with less hunger, fewer food cravings and 24% lower energy intake across test meals than placebo (Blundell et al., Diabetes, Obesity and Metabolism, 2017). It is therefore unsurprising that hunger or food preoccupation can return when that pharmacologic support recedes. This is an expected loss of drug effect, not evidence of addiction.
How much semaglutide weight regain did trials find?
The STEP 1 extension gives the most direct one-year answer. A subset of 327 participants was followed after semaglutide and the trial's lifestyle intervention ended. Those originally assigned semaglutide had lost 17.3% of body weight by week 68. Over the next year, they regained 11.6 percentage points—about two-thirds of that prior loss—leaving a mean net loss of 5.6% from the original baseline. Most measured cardiometabolic improvements moved back toward baseline (Wilding et al., Diabetes, Obesity and Metabolism, 2022).
STEP 4 tested the decision earlier. After a 20-week semaglutide run-in and mean loss of 10.6%, 803 participants were randomized either to continue 2.4 mg weekly or switch to placebo, with lifestyle intervention in both groups. Across the next 48 weeks, the placebo-switch group gained 6.9%, while the continuation group lost another 7.9% (Rubino et al., JAMA, 2021). The design cannot predict one person's outcome, but it isolates a durable pattern: removing treatment changes the weight trajectory even when lifestyle support continues.

The same pattern appears with tirzepatide
SURMOUNT-4 provides context from a different incretin medication. After participants lost a mean 20.9% during a 36-week tirzepatide lead-in, those switched to placebo gained 14.0% over the next 52 weeks. Those who continued tirzepatide lost another 5.5%. At week 88, 16.6% of the placebo group versus 89.5% of the continuation group had maintained at least 80% of the initial loss (Aronne et al., JAMA, 2024).
That is not evidence that one drug should replace the other. It is evidence that regain after withdrawal is not unique to semaglutide. Long-acting medications can help maintain a changed appetite and weight state only while their biologic effects remain present.
Why weight can return without anyone doing anything wrong
Weight loss prompts the body to defend its prior energy stores. Hunger can rise, satiety can weaken and energy expenditure can adapt. Semaglutide adds a pharmacologic counterweight by activating GLP-1 receptors and influencing appetite-related signaling. Once that support fades, the underlying pressure does not necessarily disappear.
This is why obesity is managed as a chronic, relapsing condition. Regain after stopping does not erase the work done during treatment. It shows that the treatment was carrying part of the load. The STEP 1 investigators' finding that several cardiometabolic measures trended back toward baseline strengthens the case for planning beyond the scale alone.
Is tapering better than stopping suddenly?
There is no established taper schedule proven by randomized trials to prevent weight regain after semaglutide. STEP 1 ended treatment, and STEP 4 switched participants to placebo; neither compared abrupt withdrawal with progressively lower doses. A gradual taper may be considered by a prescriber to observe appetite, side effects, glucose or weight response, but that is individualized clinical judgment—not a validated universal protocol.
The same distinction applies to maintenance dosing. Some care plans continue the established dose; others consider a lower dose or another medication. What belongs in a stopping plan
A useful plan defines what will be monitored and what would trigger another conversation. That may include appetite, weight trend, blood pressure and relevant laboratory markers. For someone using semaglutide for type 2 diabetes, glucose management needs its own plan; STEP 1 and STEP 4 enrolled people without diabetes, so their withdrawal results cannot be treated as a glucose forecast.
It also helps to decide in advance what degree of regain is meaningful. Waiting for all prior weight to return turns a manageable trend into an all-or-nothing judgment. A prescriber can instead interpret the pattern alongside tolerability, treatment goals, medical history and access. The options may include continued maintenance, a clinician-directed taper, a switch, or remaining off medication with follow-up. No single choice fits every patient, but the evidence argues for making it deliberately rather than assuming the medication's effects will persist after it clears.
