Tirzepatide units to mg is not one fixed conversion. On a U-100 insulin syringe, 100 units equals 1 mL, so 20 units means 0.2 mL of liquid — not a specific amount of tirzepatide. At 5 mg/mL, that volume contains 1 mg; at 10 mg/mL, it contains 2 mg; at 20 mg/mL, it contains 4 mg. The vial concentration is the missing number.
The patient-specific pharmacy label defines both the prescribed milligram dose and the volume to draw. The table below explains the arithmetic; it is not a dosing plan and cannot replace that label.
What units, mL and mg each measure
These three labels describe different things:
- Milligrams (mg) measure the mass of tirzepatide or semaglutide. This is the medication dose used in prescribing and clinical studies.
- Milliliters (mL) measure the volume of liquid drawn from a vial.

The word "units" can sound like a medication amount. In this setting, it is better understood as shorthand for liquid volume. It says where the plunger stops, not how much drug is in that liquid.
Published protocols make the distinction visible. SURMOUNT-1, reported in the New England Journal of Medicine in 2022, assigned tirzepatide in 5 mg, 10 mg and 15 mg weekly groups (Jastreboff et al.). STEP 1, reported in the same journal in 2021, studied semaglutide at 2.4 mg weekly (Wilding et al.). Neither trial used syringe units as the drug dose.
Tirzepatide units to mg: the formula
For a U-100 syringe, the arithmetic has two steps:
Volume in mL = syringe units ÷ 100
Medication in mg = volume in mL × concentration in mg/mL
Combined into one line:
Medication in mg = (syringe units ÷ 100) × concentration in mg/mL
The examples below use several concentrations to show why the answer changes. They are math examples, not recommendations for a dose or a statement about what any particular pharmacy dispenses.

How many mg is 20 units of tirzepatide?
Twenty units is 0.2 mL on a U-100 syringe. The number of milligrams is therefore 0.2 multiplied by the vial concentration. That works out to 0.5 mg from a 2.5 mg/mL vial, 1 mg from a 5 mg/mL vial, 2 mg from a 10 mg/mL vial, or 4 mg from a 20 mg/mL vial.
Without the concentration, "20 units" is an incomplete answer. The concentration may appear as a direct figure such as 10 mg/mL, or as a total such as 10 mg in 2 mL. In the second format, 10 mg divided by 2 mL equals a concentration of 5 mg/mL.
Why a new vial can turn into a double dose
The dangerous moment is often a refill, a new vial strength or a switch between pharmacies. Suppose an earlier vial contains 10 mg/mL and its patient-specific directions pair a 2 mg dose with 20 units. A later vial contains 20 mg/mL. Repeating the old 20-unit volume would draw 4 mg — twice as much medication. The syringe looks the same; the concentration changed.
The reverse can happen too. If a less concentrated vial replaces a stronger one, copying the old unit number may produce less medication than prescribed. That is why unit instructions should never travel from one vial to another unless the new pharmacy label confirms the same concentration and conversion. The error pattern is documented
This is not a theoretical edge case. In a 2024 alert, the FDA described compounded semaglutide errors in which patients administered five to 20 times the intended amount. Reported problems included confusing 5 units with 50 and converting incorrectly among units, milliliters and milligrams. Some patients needed medical care or hospitalization.
A 2023 Journal of the American Pharmacists Association case series reported three adverse-event cases after incorrect administration of compounded semaglutide (Lambson et al.). A larger California poison-control review published in 2025 found that 33 of 36 compounded GLP-1 exposures involved unintentional therapeutic errors; 11 were tenfold errors and five involved confusion about units of measure (Ho et al.). These reports concern semaglutide, but the same concentration-and-volume arithmetic applies to a compounded tirzepatide vial.
Semaglutide units to mg uses the same math
A U-100 syringe does not change its volume scale when the medication changes. Ten units still represents 0.1 mL and 20 units still represents 0.2 mL. The semaglutide amount comes from multiplying that volume by the semaglutide concentration printed on its own label.
What does not transfer is the dose, concentration or old unit number. Tirzepatide and semaglutide are different medications, and concentrations can differ even between two refills of the same medication. A conversion for one vial is not a conversion for another. Which number actually controls the dose
The prescriber chooses a dose in milligrams. The dispensing pharmacy prepares a known concentration and translates that prescribed amount into a volume, sometimes expressed as U-100 syringe units. Those instructions belong together:
- medication name,
- concentration in mg/mL,
- prescribed amount in mg, and
- matching volume in mL or units, plus the syringe type.
If one field is absent or the math conflicts, the prescriber or dispensing pharmacy needs to reconcile it before the next injection. A social post, an old box and an online chart cannot resolve a patient-specific discrepancy.

A mismatch is a reason to pause the calculation, not to choose the most familiar number. The pharmacy can confirm whether "units" refers to a U-100 syringe scale, whether a figure is total vial content or concentration, and whether a refill changed strength.
