Microdosing tirzepatide generally means taking less than the standard schedule calls for — staying at 2.5 mg indefinitely, or splitting a dose into smaller weekly or twice-weekly amounts. It has no clinical definition and no trial has tested it. Everything claimed about it is extrapolation from data collected at higher doses.
That is worth saying plainly before anything else, because the term is used online as though it referred to an established protocol. It does not.
What microdosing tirzepatide means in practice
There is no agreed definition, which is the first practical problem — two people describing their microdosing routine may be doing quite different things. In practice it usually refers to one of:
- Staying at the 2.5 mg starting dose rather than escalating.
- Sitting below 2.5 mg by drawing a partial dose from a compounded vial.
- Splitting a weekly dose into two smaller injections across the week.
- Extending the interval — dosing every ten days or every two weeks instead of weekly.
The motivations are consistent and reasonable: side effects, cost, a wish to maintain rather than keep losing, or a belief that a smaller dose gives most of the benefit with less of the burden.
What the evidence does and does not cover
The trials tested a specific ladder. In SURMOUNT-1, 2,539 adults took 5 mg, 10 mg or 15 mg once weekly for 72 weeks, producing average weight reductions of 15.0%, 19.5% and 20.9% against 3.1% on placebo (Jastreboff et al., NEJM 2022). The 2.5 mg dose appears in that programme only as a four-week initiation step, not as a maintenance dose anyone was studied on long-term.
So there is no trial estimate of what a year at 2.5 mg does, and none at all for sub-2.5 mg or extended intervals. Anyone quoting a percentage for microdosing is quoting a number that was measured on a different regimen.
What the data does show is a flattening curve. Going from 5 mg to 10 mg added about 4.5 percentage points of weight reduction; 10 mg to 15 mg added about 1.4. Meanwhile gastrointestinal adverse events climbed from 39% at 5 mg to 49% at 15 mg, and discontinuation for adverse events reached about 10% at the top dose (Mishra et al., J Endocr Soc 2023).
That shape is the strongest honest argument in the vicinity of microdosing: more is not proportionally better, and less is not proportionally worse. It is an argument for a lower dose, not evidence for a microdose.
The pharmacology problem with splitting and spacing
Tirzepatide's half-life is about five days, which is what makes weekly dosing work — levels stay reasonably steady between injections.
Splitting a weekly dose into two smaller ones does not lower your average exposure; it lowers the peak and raises the trough. Some people find that easier on the stomach, which is plausible given that side effects track the post-injection peak. It is a redistribution, not a reduction.
Extending the interval is the opposite. Dosing every two weeks lets levels fall substantially between doses, which can reintroduce the escalation-phase side effects each time you dose — the pattern that makes the first weeks hard, repeated indefinitely.
The better-supported version of what microdosers want
If the goal is fewer side effects, there is a strategy with direct evidence behind it: slow the titration. In a 2025 randomized pilot on the closely related GLP-1 semaglutide, gradual flexible titration cut gastrointestinal-related withdrawals from 19% to 2% and nausea from 64.2% to 45.1% versus the standard schedule, without compromising outcomes (Eldor et al., Diabetes Care 2025; a published correction was issued in February 2026).
If the goal is a sustainable long-term dose, settling at a studied maintenance dose is the evidenced path. 5 mg is a recognised maintenance dose and produced 15.0% average weight reduction in SURMOUNT-1 — most of what 15 mg delivered, at 39% versus 49% GI adverse events. Choosing 5 mg over 15 mg is not microdosing. It is dosing, at the low end of a range that was actually studied.

Where the real risk sits
The risk in microdosing is rarely the small dose itself. It is the do-it-yourself measurement.
Compounded vials are prepared at a concentration the pharmacy sets, and instructions are often written in units on an insulin syringe. Units measure volume, not drug — so drawing "half" from a vial whose concentration you have not confirmed is a guess dressed as arithmetic. None of that is an argument against a low dose. It is an argument against improvising one without telling anyone.
What to do instead of quietly microdosing
A prescriber can write a low dose. That is an ordinary clinical decision, and holding someone at 2.5 mg or moving them back down is a normal response to side effects or to a maintenance goal. The difference between that and self-directed microdosing is that one is documented, monitored and adjustable, and the other is invisible to the person responsible for your care.
