Wegovy prescribing information is the official label for branded semaglutide. In plain English, the current injection label covers three uses, a gradual dose ladder, two firm contraindications, serious warnings, and the side effects seen in trials. It also includes the newer 7.2 mg Wegovy HD dose. The label is a reference, not a personal plan: a prescriber sets the actual dose.

Wegovy prescribing information: what is current

As of September 18, 2026, the day this article was written, DailyMed lists the current Version 19 record as published June 30 and effective June 18, 2026; it says Revised: 6/2026. The record combines the injection and tablet labels; this page focuses on the injection.

The current label includes single-dose pens and syringes, Wegovy HD, and a four-dose FlexTouch pen. The agency's supplement letter for the 7.2 mg dose is signed March 19, 2026. It adds that dose for adult weight reduction and long-term maintenance, not for every Wegovy indication.

What Wegovy injection is labeled to treat

An indication is a use written into the label. Wegovy injection has three: long-term weight reduction in adults and adolescents 12 and older with obesity, and in adults with overweight plus a related condition; reduction of major cardiovascular events in adults with established heart or blood-vessel disease and overweight or obesity; and treatment of noncirrhotic MASH, liver inflammation with moderate-to-advanced scarring, in adults.

The cardiovascular wording rests on SELECT. In that 17,604-person trial, cardiovascular death, nonfatal heart attack, or nonfatal stroke occurred in 6.5% with semaglutide and 8.0% with placebo (Lincoff et al., New England Journal of Medicine, 2023). For MASH, the 2025 ESSENCE interim analysis found resolution without worse fibrosis in 62.9% versus 34.3%; that indication uses accelerated approval while a confirmatory trial continues (Sanyal et al., New England Journal of Medicine). Those are trial results, not forecasts for one person.

The dose ladder is a label fact, not a plan

The injection schedule below reports the label. It is not an instruction to self-dose.

Label stage Weeks Once-weekly injection
Starting dose 1–4 0.25 mg
Escalation 5–8 0.5 mg
Escalation 9–12 1 mg
Escalation 13–16 1.7 mg
Maintenance 17 onward Depends on indication and tolerability

For adult weight reduction, the label lists 1.7 or 2.4 mg as maintenance doses. It says 7.2 mg may be considered only after 2.4 mg has been tolerated for at least four weeks and more weight reduction is clinically indicated. The 72-week STEP UP trial compared 7.2 mg, 2.4 mg, and placebo; gastrointestinal reactions occurred in 70.8%, 61.2%, and 42.8%, respectively (Wharton et al., Lancet Diabetes & Endocrinology, 2025).

The missed-injection rule turns on 48 hours: with more than two days before the next scheduled dose, the label allows the missed dose; with less than two days, it skips that dose and resumes the regular day. After two or more missed weekly doses, the label returns the restart decision to the prescriber.

The boxed warning and contraindications

A boxed warning is the label's most prominent safety warning. Semaglutide caused thyroid C-cell tumors in rodents. The label says it is unknown whether that finding applies to humans.

A contraindication is a reason the branded drug must not be used. Wegovy lists two: a personal or family history of medullary thyroid carcinoma (MTC), a rare thyroid cancer, or multiple endocrine neoplasia syndrome type 2 (MEN 2), an inherited tumor condition; and a previous serious allergic reaction to semaglutide or an ingredient in Wegovy.

Wegovy warnings and precautions in plain language

The main warnings are easier to read as a set:

  • Pancreatitis means inflammation of the pancreas; the label flags persistent or severe abdominal pain.
  • Gallstones and gallbladder inflammation occurred more often with Wegovy than placebo.
  • Hypoglycemia means blood sugar that is too low. Risk rises with insulin or a sulfonylurea, a diabetes pill that increases insulin release.
  • Vomiting or diarrhea can cause dehydration and acute kidney injury, a sudden loss of kidney function.
  • Rapid glucose improvement can temporarily worsen diabetic retinopathy, damage at the back of the eye, in people who already have it.
  • Resting heart rate rose by an average 1 to 4 beats per minute in adult injection trials.
  • Pulmonary aspiration means stomach contents enter the lungs during anesthesia or deep sedation. The label says planned procedures belong in the medical-team conversation; semaglutide before surgery covers that decision separately.

The June label no longer contains the earlier warning about suicidal thoughts or behavior. Its change log marks that section Removed 02/2026. This matches the FDA's January 13, 2026 review, which found no increased risk and requested that the warning be removed.

Adverse reactions in the current label

Adverse reactions are side effects recorded during studies. For the two 72-week Wegovy HD trials, the label reports:

Reaction Placebo 2.4 mg 7.2 mg
Nausea 13% 35% 39%
Vomiting 6% 16% 22%
Dysesthesia, altered skin sensation 0% 6% 22%
Constipation 8% 19% 20%
Abdominal pain 7% 9% 12%
Fatigue 5% 9% 11%

These percentages describe trial groups. They do not predict one person's experience. The broader semaglutide side-effects guide covers the usual timing and less common risks.

Pregnancy and other specific populations

For weight or cardiovascular use, the label says treatment is discontinued when pregnancy is recognized. Its preconception direction is exact: “discontinue WEGOVY at least 2 months before a planned pregnancy because of the long half-life of semaglutide.” MASH requires a separate risk-benefit judgment. Semaglutide and pregnancy explains the limited human evidence without softening that label direction.

The label also separates injection from tablets during breastfeeding because the tablet contains an absorption helper. It reports no overall safety difference in adults 65 and older, though fewer people were 75 or older. No injection dose adjustment is listed for kidney or liver impairment, but dehydration-related kidney injury remains a warning.

How Wegovy injection is supplied

The label lists single-dose pens from 0.25 to 7.2 mg, single-dose syringes from 0.25 to 2.4 mg, and a single-patient FlexTouch pen containing four 2.4 mg doses. Single-dose devices are refrigerated at 36°F to 46°F and may remain at 46°F to 86°F for up to 28 days before cap removal. The FlexTouch has different after-first-use storage rules, so the exact device label matters.

What this label cannot decide

A label organizes evidence for one branded product. It cannot decide whether semaglutide fits one person's history, whether another option such as tirzepatide is a better fit, or how a separately compounded prescription is prepared. Those are clinical and product-specific questions.

What the label says about compounded semaglutide

The Wegovy label does not establish the concentration, container, storage, or directions for a compounded preparation. Through Promise, semaglutide is dispensed as a compounded medication, which is different from an FDA-approved product: the formulation offered here is not FDA-approved.

A licensed provider may still prescribe a compounded formulation; that decision is between the patient and the doctor. At Promise, a licensed provider reviews every request and not everyone qualifies. The pharmacy label and the prescriber's directions govern the preparation actually dispensed.
